Send directions to your phone from Google Maps on the web
If you're signed in to the same Google account on both your computer and phone, you can just select Send driving directions to your phone that appears on the left under the destinations in the Google Maps window on your computer.
You will receive a notification that takes you directly to navigation in Google Maps on your device.
